SHBG (Sex Hormone-Binding Globulin) Test: What It Measures and Why It Matters

SHBG is a protein made by the liver that binds testosterone and estradiol in your blood and controls how much is available to your body.

What the SHBG test measures

Why we check it at Serene

SHBG explains why hormone numbers and symptoms don’t always match. High SHBG ties up more hormone, so you may have symptoms even with a “normal” total testosterone. It’s also used to calculate free testosterone.

At Serene Med Spa in Barboursville, West Virginia, every hormone and wellness plan starts with Labcorp lab work reviewed by a board-certified physician.

What your results can mean

Higher than normal

SHBG tends to rise with age, higher estrogen, an overactive thyroid, liver conditions and some medications.

Lower than normal

Low SHBG is often seen with insulin resistance, excess weight, an underactive thyroid and, in women, PCOS.

Normal ranges vary by lab, age and sex. Your physician reads every result together with your symptoms, history and other tests.

How to prepare

No fasting is needed. If you take biotin (vitamin B7) or a hair, skin and nail supplement, tell us first. The FDA warns that high-dose biotin can interfere with some lab tests, including hormone and thyroid tests.
